Database fields 

I am trying to integrate a payment processor into DNN.

The information their script needs is:
Database name
Table Name
Database Username
Database Password
and
Username / Field Name / Password Field Name
Where can I get this information?

Well the database name, database username, database password are all stored in your connection string that was setup in your web.config whn you installed the site. 

As for the other items, I assume that they are looking for the table name and fields that store the users information.....I would consult the payment processor for the specifics of what they are looking for.
