Navigation and Pages 

I have a requirement to dynamically change the navigation when the user logs on.  The user has the option to setup his profile so that he or she can monitor other people’s conditions.  For example a user might have two children with one having Diabetes and the other with High Blood pressure.  Based on their profile the nav should look like this

Home
Profile
Grant Access
John Doe
--Monitor Conditions
--Shared Treatment Plan
--Reports
Chris Doe
--Monitor-Conditions
----Diabetes
--Shared Treatment Plan
----Diabetes
Krista Doe
--Monitor-Conditions
----High Blood Pressure
--Shared Treatment Plan
----High-Blood-Pressure

I need to be able to dynamically create the Pages for each user in the profile so that it will show up in the navigation correctly.
