I personally manage DNN in an environment like this on a day to day basis, and have trained many different companies on how to do it as well.  As metioned previously it takes a bit of work to get it right, but it can be done.

The hardest part is working with content that needs to be pushed from Dev -> Test -> Production.  All other items are pretty easy to manage.

Yes I suppose it was the page content, layouts, etc, that I was most curious about. I'd want to prototype and finalise changes to content on a development platform, then migrate it to the live platform. I have pretty good SQL Server support - is the content easy to migrate independently of the data? (e.g. migrating new and modified page designs from development to live databases while leaving user-entered data (e.g. survey results) intact? I'm not familiar with the design of a typical DNN database, as you have probably guessed!
