DNN provides a Feedback module default but is more like a singular comment field and may not be a "custom f0rm" like you are looking for.  Personally I have used either XMOD or Data Springs, Dynamic f0rms to do custom f0rms.  Both are excellent modules in general for your collection.  The learning curve is much smaller on Dynamic f0rms for your particular purpose.  Unfortunately neither of those are free.  I'm not aware of a free module but one may exist out there others can point you too or you could of course program you own if that is in your competency.
