Can I add questions i.e. textboxes? 

Hi,

Does DNN feedback or survey modules give me the ability to add questions i.e. textboxes? I want to have a 5 question feedback/survey on the site. The answers will get emailed to me so it's a very basic set up.

Does DNN give me this functionality out of the box or do I need to get a third party module?

you can modify feedback module by adding additional text boxes or wait for next Survey module version (5.0) which has been announced to support text boxes as well.

Meanwhile, you can use any available forms module, e.g. free "Helferlein form" from www.helferlein.com
