Resend & Subject Line Bug in Feedback Module

DNN Version:  4.3.5

Feedback Module Version:  3.02.01

Bug
: The feedback module ignores the subject line and sends duplicate e-mails upon using the same feedback form a second time.

How to Duplicate:  On any page, add a new Feedback Module (I normally delete the accompanying comments module immediately afterwards). 

Go to the settings for the Feedback Module and put in a subject line of your choosing, and enter in an e-mail address in the "Send To" field that you can check.  Make sure that it is an alternate e-mail addres than what you will be using to send comments with.

Log out.

If needed, navigate back to the page with the new feedback module.  Enter your from e-mail address, name and a message.

Click send.

Proceed to your inbox for the e-mail address that the e-mail was supposed to be sent to.  Everything should appear to have worked like normal.  DO NOT navigate to another page or click anything else on the page with the feedback module.

Once you have verified that you received the previous e-mail, enter you e-mail address and name again, but enter a different message in the comments field and click send again.

You should now receive two e-mails in your Inbox.  Neither e-mail will have the subject line, and the from e-mail address will also have the same exact e-mail in its inbox even though we did not enable this feature (which is a spoofing security risk that I disable, by the way).
