Issue with DNN 4.5 and Latest Survey

We're getting a strange issue with the 4.5 RC3 and the Survey Module.  Setup a simple one question, two answer survey.  Set to public.  Logout and select an answer, click submit and get this displayed below the survey:

The survey is incomplete. Please complete and resubmit.

Seems like a validator issue.  There is nothing in the Event Log.  Chaning all settings does not change the behaivior.

Since I'm new to DNN what is patch release techiques and procedures.  Where / can I get the patch to update DNN 4.5.0?
