What are the localization  settings for the site and the user?

If you are not using US-English, can you change this around so we can confirm or rule out this as a cause?

It is set to US-English.  All users are set to this and it sends the email error to me (admin) for everyone who posts.

I also have the same issue. The Portal's email configuration is set up correctly, and the administrator's email is valid. I am running v3.0.13 with the latest version of the forums. Until today I was running v3.0.12, I was reading this post because my emails just werent working but there was no CDO error message displayed. After upgrading to .13 today the CDO message has started to display.

Any leads on a cause?

I am having difficulties reproducing the error.  I can say that from .12 to .13 (DNN) the email column was added back to the Users table.  This is why the notification was able to start working.

Its not the the email is NOT being sent.  It is being sent out correctly, but rather that a CDO error is then displayed to the user and the Admin receives a CDO error message email!

Strange, but funtional...Anoying as hell :) hehe
