Survey 04.00.70 Ready for release (DNN 4.5 compatible)

The 04.00.70 version of the Survey module is ready for release. It fixes these two bugs:

SUR-5176 - Pressing F5 allows you to vote twice
SUR-5363 - Cannot Submit Survey in DNN 4.5

A special thank you goes out the the numerous Core members who helped get this release out as fast as possible even though they have a lot of of work and issues to deal with right now.
