It was a long way to get here, but now I am very pleased to announce that Form and List was submitted to the release tracker. Form and List is the official successor of UserDefinedTable3.5.1, created for you by the UserDefinedTable project team.
The UserDefinedTable evolved over the last years, just as DotNetNuke. Starting as an elementary customizable table, UserDefinedTable turned into a feature rich list tool. UserDefinedTable now jumps over directly from DotNetNuke 3 and ASP.NET 1.1 to DotNetNuke 5 – Cambrian, and turns into something new. We decided to change the name to "Form and List", pointing to the new opportunities instead of module's architecture.
The module introduces new possibilities; most important and name giving is the new ability to create submission forms as primary view for the module:
You can access the submissions as list view directly from module action.
The preferred usage is to mirror the module instance at a second page and to apply a list view for that tab:
The new module comes with a set of new features and enhancements, and I am going to introduce them over the next time to shorten the waiting period for the public release.

Oh, cool. Thanks for allowing comments without moderation. In my book, this is the right way to go. In the new wonderful world of Web 2.0, it's one less obstacle in the way of good conversation at internet speed!

Is the interenal name also changed ? i.e. would it installed as a new Module or would it upgrade the existing UserDefinedTable ?

Form and List will upgrade existing UserDefinedTable modules.
Form and List is now used everywhere in UI, and all friendly names are replaced with the new term. So normal users will only see "Form and List". I can't change the name everywhere without introducing breaking changes, so the old name remains internally. The namespaces for code and XML are still using UserDefinedTable, and the module still remains in the old folder.

Very nice, so often I have clients asking for a simple to use free form they can put up on their site. The UDT was a great tool but was not simple enough for the average user. From the looks of it this will provide what a bunch of my clients want nearly out of the box. Thanks for the work. I can't wait for DNN 5.0 and all of its accompanying updated modules. Everyone has been doing such an awesome job making DNN the platform it should be. :)

This is very exciting! Just yesterday I was using UserDefinedTable wishing it was more like a SharePoint list object. Looks like you were already well ahead of my wish!

At last. Out of the box a core module that allows you to build forms. Although I thought this functionality would come from a different source - the feedback module. After reading info back in the summer on v5.0 of the Feedback module (which at that time was made available for testing but has not yet - to my knowledge - been released) it looked like the Feedback module would become the de-facto form-creator for DNN. With this upcoming release of the Form and List module I think it time to do a critical review of the overlap of the two modules. My guess is that, with a few tweaks, the Form and List module will be able to do much - if not all - the Feedback module currently does (and aspires to do in the unreleased v5.0)

Will it be possible to let the module calculate "overall" figures, like the total amount of some kind? Or, for that matter, let information in one form be dependend of this totals? I could build workflow functionality or stock calculations then. Anyway, I think this is a great module with lots of potention! Keep up the good work! Cheers, Jos Richters

Jos, this is nothing new: Calcluated Column allows you to calculate the sum, and I posted a blog about a running sum some time ago. XSL enables you also to return content depending on values or results of operations.
