Form and List 05.00.00 is the official successor of UserDefinedTable3.5.1, created for you by the UserDefinedTable project team.
As already mentioned in my last blog, Form and List allows you to create submission forms. Submissions are stored inside the module. Most times you want to get informed about new submissions, therefore the module you to configure your desired notification scheme. Form and List is able to send email notifications, triggered by all change events: on new, on update and on delete. Notification doesn't differ on the current selected mode; it works both for forms and lists.
The Recipients can be set to Email addresses, or can be retrieved from the data. Available sources are columns of data type Email and also columns of data types Link to User Profile, CreatedBy and ChangedBy, which will look up the email address for the specific user.
The message is customizable. By default, you can define an introducing text for the message, followed by text specific for the current trigger. Afterwards the input is added as a transposed table.
If you need more customization, you can define the whole body using XSL. Self- evident to say that the Token Based XSL Editor and Generator helps you here the same way as you are used to generating XSL stylesheets for Lists.
The settings for notifications are per page instance, you can apply different kind of notifications including the message text on different pages.
Test it on your own
I am excited to inform you that the current package, while still on its way through the release process, is included in release candidate 2 of DotNetNuke 5.0. One part of the module release process is normally stress testing and dogfooding, which can't get done without existing sites running DNN5.
So if you like, download DNN5 RC2 and test it together with the new module. Please share your impressions, tell us what you like or not. Please report any bugs to the UDT forum.
However, users need to be aware that some serious bugs may yet surface and this distribution is NOT RECOMMENDED FOR PRODUCTION USE at this time.

I just did some testing and I must say, I really like this module. Even my clients will be able to figure out how to make a form now. It is so nice to finally have a core module that allows users to create forms.
A couple ideas you might consider (mabe you have) is to redirect to URL or Page upon submit and add user to role upon submit.
Congratulations on making such a great module and I personally thank all of you who have contributed.

I fully second Mike's suggestion to redirect to URL or Page upon submit. It would be great if the Form and List could get this into the next release.
