try enabling file sync.   Host Settings / Advanced Settings / Other Settings / Auto-Sync File System? (Check to enable).  You must be logged in to host account to change this setting.

I'm getting this as well, in a recent upgrade to 4.5.3.

I've done a lot of cleanup in the files/folders area and made sure all the newer constraints have been applied to the files and folders tables. I get this error whenever I schedule file system synchronize ... even after I go to each and every portal admin | file manager and do a manual resynch recursive (no errors and folders lastupdated column is getting some updates).

What would be causing this error?

In my case, it was the way I installed PageBlaster ... if you are using PageBlaster, and it is using the /Portals/_default/Cache folder as its cache-folder, then create a "Cache" folder at the DNN root, and alter the pageblaster config

        cache-folder="/Cache"

this is for the free version of PageBlaster. That took care of all my problems.

Edit: Additionally, I deleted the folders below the /Portals/_default/Cache folder which were created by PageBlaster as well as any pb.webinfo file in /Portals/_default/Cache ... not visible in FileManager, had to do this at the filesystem level.
