Thanks for your answer Cris Hammond but on Downloads page next to Survy 4.05.00 file in "Min DNN Version" column there is written 4.3.5  !?

Does this means that 4.3.5 is minimum version or is this April 1st joke ?

Anyway it works fine for me (after applying above mentioned trick) on dnn 4.3.5. !

This was my fault. I have requested that the page be updated.

I am using DNN 4.5.1.  I just installed the survey 4.5 version.  When I went into settings, here was the error I received:

Unhandled error loading module.

Anyone know what I need to do in order to get my surveys back running the way they used to before I installed this "upgrade"?

You should be able to re-install the previous version of the Survey module to fix it.

I now realize that the minimum version of DotNetNuke for the latest Survey module is 4.5.2. I will have them update the downlods page.
