Just curious - who is hosting your site?  The hosting company I'm with provides an interface for installing the latest version of DNN.

This is a version compatability problem.

Version 3.0.13 of DNN is no longer supported by the Survey module.

If you are on that version, you can get the Survey Module that came with that version and it will work though.
