Accepted Answer 

Turns out the problem was the user put this in their web.config (to solve an unrelated problem):

<location inheritInChildApplications="false">
      
</location>

Removing this fixed the problem.

Michael, Thanks for your post. This fixed my problem of not being able to upload any modules.

However, now my sub Applications that relied on te location tag in web.config doesnt work. Is there any way to have my cake and eat it too?

I'm in the same boat as Carlos.  While i could remove the section from web.config to get the survey module working, what about the other modules that need that part of web.config?  There has to be a way to make the survey module work without breaking the other modules on the site.
